When a resident chooses Hibbing HRA to fulfill their housing needs, they also make a personal choice for growth and success. Those that are between the ages of 18-62 are automatically enrolled in the Empower Program and set up with a plan of what success looks like to that individual resident. This involves pairing residents with a service coordinator to do individual case management, annual needs assessment, referrals to services based on needs identified in the assessment, follow up and providing education on topics of self-sufficiency.

Service Coordinator Support

  5. Evaluation: Reviewing outcomes to determine success and identify areas for improvement.
